    When I was a young architect at a new job, I was advised by a senior technician to write down and date every verbal instruction I received from the boss as he had a habit of denying he’d said something. I’ve been doing it ever since at every place I’ve worked.

    Yup, document decisions from the entire team, especially if you’re the spec writer. People change their minds: the client, the designer, and sometimes even the manufacturer / supplier will change. Oh, and the singular is “Addendum”.

    yup, I keep all my mark-ups / instructions in a CMA (cover-my-ass) file. It has been useful on more than one occasion…
